Service Coordination
Service Coordination assists people with developmental disabilities gain access to services and supports appropriate to the needs of the individual. It is provided by qualified service coordinators and uses a person-centered planning process in developing, implementing, and maintaining an Individualized Service Plan.
Services
Service coordinators help individuals and families navigate the system of disability services. They facilitate meetings, research services, identify appropriate services, advocate for their participants, and much more.

Eligibility/Funding
Individuals must have a developmental disability and live in Onondaga County to be eligible for service coordination. Funding is provided by Medicaid and/or local assistance grants from Onondaga County.
