 Individualized Living Services
Individualized Living Services provides support and assistance to people in their homes and neighborhoods. This service promotes choice as well as individualization. Habilitation Specialists act as both a role model and connection to the community. This service empowers people to have control over their own lives.
Services
Supports we provide are tailored for the individual and may include training in the following:
- Community Inclusion
- Money Management
- Health Promotion
- Home Safety Skills
- Housekeeping
- Use of Transportation
- Problem resolution
- Communication
- Time Management
- Self Advocacy and Improvement
- Personal Hygiene and Grooming
- Diet and Exercise
Eligibility
To participate in this service an individual must have a developmental disability and be approved for funding through the Home and Community Based Services Waiver (HCBS). The person must already have approval from the local DSO to receive Residential Habilitation at Home, one of the components of the HCBS waiver.
Funding
Individualized Living Services are funded under the HCBS waiver, and must have prior approval from the local Developmental Services Office to receive Residential Habilitation.
