Main Function
Provide training and facilitate learning of personal and social skills necessary for activities of daily living and community inclusion within a residential setting.
Duties
- Implementation of Goal Plans/Habilitation Plans
- Contributes to development and review of Individual Goal/Habilitation Plans.
- Provides training and assistance in area of ADLs with resident as designated in the Individual Plan (including bathing, grooming, toileting, dressing, laundry, etc.).
- Carries out responsibilities as assigned by the Program Manager (i.e., doctor's appointments, church attendance, banking, counseling sessions, monthly summaries, goal plans, etc.).
- Foster independence and autonomy.
- Supervision of Medications
- Completes Medication Certification Course, as required.
- Follow AMAP policy as directed.
- Community Integration
- Provide updated information and access to neighborhood/community resources aimed at full "inclusion" to the consumer.
- As opportunities arise, expose consumer to a variety of functions/activities to increase choice making.
- Communicates with Program Manager in regard to implemented integrated activities and any issues that may arise.
- Crisis Management
- Utilizes pro-active approach to avert crisis situations.
- Utilizes on-call supervisor to test perceptions and request emergency assistance.
- Professionalism
- Participates in regular staff meetings.
- Develops an appropriate rapport with residents.
- Keeps written and verbal information shared with team members regarding a resident confidential.
- Demonstrates sensitivity and responsiveness to residents and respects confidentiality needs.
- Works with supervisors to meet program needs.
- Works with other service providers in the community in a professional manner.
- Wears attire appropriate to the working environment and situation.
- Participates in Quality Assurance Program and Total Quality Management Teams as assigned.
- Documentation
- Documents all paperwork in a clear, objective and professional manner (i.e., Incident Reports, 24 hour reports, weekly summaries, program information, etc.) in a timely manner.
- Basic Health and Safety
- Completes certification in C.P.R. and First Aid.
- Is oriented to fire procedures and disaster plan.
- Maintains home so as to ensure safety and cleanliness on a daily basis.
- Maintain AMAP Certification.
- Meal Planning and Preparation
- Trains and assists residents in meal preparation to promote skill building and independence.
- Transportation
- Reports any accidents or traffic violations, minor or major, immediately to Program Manager or Supervisor on-call.
- Drives according to N.Y.S. laws.
- Uses company vehicles in a responsible manner.
- Uses company vehicle for job-related business only.
- Housekeeping
- Completes daily housekeeping tasks with assistance from residents.
- Completes Other Duties as Assigned
Scope of Responsibilities
With on-going supervision, provide training and assistance to individuals living in a community based IRA/ICF with the intent of encouraging independence and inclusion. As we provide total supervision, responsibilities also include ensuring 24 hour protective oversight to the individuals we serve.
Education and Experience
High school diploma/GED. Experience working with people with disabilities preferred.
Conditions of Employment
Must have a valid NYS driver license in accordance with the Agency Vehicle and Driving policy. Must be able to lift 75 pounds. Must be able to work overnight shifts as assigned by supervisor. Successful completion of required in-service training. Must participate and meet established requirements in a database check through the Division of Criminal Justice Services as defined by the State of New York, Office of People With Developmental Disabilities, Chapter 575 of the Laws of 2004.
