Main Function

Manage the day-to-day operations of a program component within the Community Services Department. Supervise a team of employees providing Medicaid Service Coordination (MSC).

Duties

  1. Integrity
    • Adhere to ethical principals including the Agency’s Mission Statement and Standards Of Conduct.
    • Oversee the provision of quality services through observation, ongoing supervision of staff and communication with participants and their advocates.
    • Schedule staff to maintain services for planned and unplanned absences, personally providing coverage when necessary.
    • Instruct MSC’s on protocols for conducting ISP meetings.
    • Assist supervisor in determining program requirements. Communicate changes prior to implementation.
  2. Respect
    • Recognize, encourage, and support opportunities that promote natural connections within each participant’s community.
    • Identify and articulate participant’s needs and desires in a written habilitation plan consistent with their ISP which includes goals, strategies and methods to implement.
  3. Teamwork
    • Initiate and maintain ongoing communication with participant and key people integral to achieving goals and providing services.
    • Participate in Agency work groups for quality improvement, fund raising and/or strategic planning.
    • Facilitate staff meetings. Share pertinent service, departmental and Agency wide information.
  4. Professionalism
    • Keep written and verbal information shared with team members regarding a participant confidential.
    • Work with other service providers within the community in a professional manner.
    • Role model appropriate dress and behavior at work.
  5. Accountability
    • Complete documentation based on program requirements following established guidelines and timeframes.
    • Ensure consistency between ISPs and complimentary service plans and assess effectiveness of plans.
    • Instruct MSCs in proper methods for monitoring plan compliance.
    • Utilize pro-active approach to avert crisis situations.
    • Work with the Human Resource Department to recruit, interview, hire and train staff.
    • Provide ongoing direction, support and performance evaluation of assigned staff.
    • Document performance concerns and follow-up with disciplinary action if necessary.
  6. Innovation
    • Research community resources and provide linkage to increase inclusion with the participant’s community.
    • Foster independence and autonomy.
  7. Manage On-Call Responsibilities
    • Share 24 hour on-call beeper rotation.
  8. Perform Other Duties as Assigned

Scope of Responsibilities

The Program Manager works independently and is able to use personal and professional discretion in implementing new approaches and/or problem solving. With minimal supervision, prioritize outcomes, provide training and interact with participants in the community. Contact with the public is moderate. Errors can have serious consequences for assigned participants and/or staff.

Education and Experience

Bachelor’s degree (Human Service field, preferred) plus one year experience with people with disabilities required. Completion of Self Determination broker certification within 90 days of employment is required. One year supervisory experience preferred. Basic work processing and e-mail skills are required.

Conditions of Employment

Must have a valid NYS driver license in accordance with the Agency Vehicle and Driving Policy and transportation necessary to meet program needs. Schedule flexibility to accommodate participant needs, which may include evenings and weekends. Must be able to lift 75 pounds. Within 3 months of employment, must attend and maintain CPR, First Aide, RIT, and Promoting Positive Interactions trainings. SCIP-R training as necessary. Must participate in database check through the State Central Registry of Child Abuse and Maltreatment.  Must participate and meet established requirements in a database check through the Division of Criminal Justice Services as defined by the State of New York, Office for People With Developmental Disabilties, Chapter 575 of the Laws of 2004. Must participate in successful database checks through the HHS Office of Inspector General, Excluded Parties List System, New York Office of Medicaid Inspector General and Specially Designated Nationals.
