Main Function
Manage the day-to-day operations of a program component within the Community Services Department. Supervise a team of employees providing direct services.
Duties
- Manage Daily Operations
- Oversee daily operations in keeping with agency policies and budget guidelines.
- Schedule staff. Arrange, coordinate, and personally provide coverage when necessary.
- Coordinate with family members/advocates and other Provider Agencies involved with assigned participants. Communicate regularly with Program Coordinator, Service Coordinators and relevant clinical staff.
- Supervise Assigned Staff
- Work with the Human Resource Department to recruit, interview, and hire qualified staff.
- Orient, train, and evaluate assigned employees.
- Train staff regarding implementation of individual day habilitation plans.
- Provide ongoing direction, support and performance evaluation of assigned staff. Write initial evaluation period review and annual performance reviews.
- Document performance concerns and follow-up with disciplinary action if necessary.
- Facilitate regularly scheduled staff meetings. Share pertinent service, departmental and agency wide information. Encourage staff participation.
- Develop and Implement Individualized Plans
- Coordinate assessments.
- Facilitate Annual and Semi-Annual meetings.
- Listen to participants needs and wishes.
- Write individual plans and goals according to OPWDD and Agency procedures. Revise plans as needed.
- Maintain files of assigned participants.
- Ensure the well-being and safety of participants.
- Monitor Service Delivery
- Maintain daily progress notes.
- Relate activities to goals identified in participants' individualized plans.
- Provide appropriate documentation for billing to the Business Office.
- Utilize proper forms and procedures for documentation.
- Prepare quarterly and annual reports.
- Foster Community Inclusion
- Design activities to maximize community participation and integration of program participants with people who do not have disabilities.
- Provide linkage to increase natural supports.
- Research community resources.
- Support participants to use a variety of modes of transportation, including public transportation.
- Demonstrate Teamwork & Professionalism
- Attend and participate in Expanded Management and Department meetings.
- Role model appropriate dress and behavior at work.
- Represent Agency positively to other agencies and the community.
- Participate in Agency work groups for quality improvement, fund raising and/or strategic planning.
- Manage On-Call Responsibilities
- Be available to respond to emergency situations as they arise.
- Perform Other Duties as Assigned
Scope of Responsibilities
The Program Manager works independently and is able to use personal and professional discretion in implementing new approaches and/or problem solving. With minimal supervision, prioritize outcomes, provide training and interact with people in the community. Contact with the public is moderate. Errors can have serious consequences for assigned participants and/or staff.
Education and Experience
Bachelors degree in human service field plus one year experience with people with disabilities preferred or Associates degree in human service field plus two years experience with people with disabilities required. Supervisory experience preferred. Basic word processing and e-mail skills are required.
Conditions of Employment
Must have a valid NYS driver license in accordance with the Agency Vehicle and Driving Policy and transportation necessary to meet program needs. Schedule flexibility to accommodate participant needs. Must be able to lift 75 pounds. Must be willing and able to provide direct care service, including personal assistance. Within 3 months of employment, must attend and maintain CPR, First Aid, SWAP, and Promoting Positive Interactions. SCIP-R training as necessary. Must participate and meet established requirements in a database check through the Division of Criminal Justice Services as defined by the State of New York, Office of People With Developmental Disabilities, Chapter 575 of the Laws of 2004
