Social Work Coordinator (LCSW-R)

Main Function
The Social Work Coordinator is responsible for administrative and clinical leadership to social workers in the Comprehensive Clinical Services department. The Social Work Coordinator will also provide diagnosis, psychotherapy and treatment to enrolled/referred participants and their families/caregivers as appropriate.
Duties
1. Administrative Coordination
§ Supervise staff of Social Workers in the Comprehensive Clinical Services department.
§ Oversee case referral and assessment to the Social Work discipline for individual or group counseling or for STARS (social skills) training.
§ Coordinate the staffing and other resources necessary for STARS training.
§ Convey information to and from the Agency leadership and the Social Workers.
2. Social Work Services
§ Explain paperwork and processes as needed to participants and their families focusing on those without service coordination.
§ Assist individuals/families with "transitions" between and among different services/departments within Enable as well as outside services.
§ Provide ongoing communication and advocacy for participants with complex needs, simultaneous multiple services and/or equipment orders.
§ Assist in facilitating insurance approval and payment.
3. Psychotherapy and Counseling
§ Provide assessment and individual or group and family counseling for participants or their family members.
§ Assist individuals and families with goal-setting and problem solving.
§ Deliver STARS (social skills) training for youth.
4. Information, Referral & Advocacy
§ Provide information and resources (people, books, websites) regarding diagnoses, disability and treatment issues.
§ Share contact information regarding Enable and other agency services.
§ Refer participants and/or their families to community resources to address their comprehensive needs, such as: financial assistance, legal aid, transportation, housing, health care, counseling, service coordination, job placement or education.
§ Consult with interdisciplinary team and other agency service providers to address barriers to access and improve overall service outcomes. Advocate for participants/families to resolve concerns, or direct to appropriate advocacy channels.
5. Documentation
§ Complete appropriate clinical documentation consistent with applicable regulations, including HIPAA.
§ Complete appropriate clinical documentation consistent with applicable regulations for use in obtaining payment from public or private payer sources.
6. Community Involvement
§ Represent Enable within the community as assigned.
7. Performs other duties as requested.
Scope of Responsibilties
With minimal supervision, following Agency policy and procedure, professional standards and applicable regulations, uses own experience and judgment to assist participants and their families. Broad knowledge of community services and resources in the Syracuse/Central New York area required. Position has access to confidential information which must be respected.
Education and Experience
Master's degree in Social Work required. LCSW-R required. Recent experience in relevant social work field and experience in working with individuals with disabilities strongly preferred.
Conditions of Employment
Must have a valid NYS license in accordance with Agency Vehicle and Driving policy and transportation necessary to meet program needs. Must have moderate-level computer skills - must be able to utilize electronic medical record when available. Must be able to lift 15 pounds. Must complete a pre-employment physical (Post-Offer) which is sufficient in scope to ensure that the employee is free from a health impairment which is of potential risk to participants or which may interfere with the performance of his/her duties as required by the New York State Department of Health (Article 28). A Health Assessment form must be completed annually thereafter. Documentation sufficient to provide credentialing with Medicare and private insurances must be provided. Must meet continuing competency requirements. Must participate in successful database checks through the State Central Registry of Child Abuse and Maltreatment, HHS Office of Inspector General and Excluded Parties List System. Scheduling flexibility to accommodate evening meetings and activies.
