Main Function
Provide training and facilitate learning of personal and social skills necessary for activities of daily living at home, work and in the community.
Duties
1. Implementation of Goal Plans/Habilitation Plans
- Follow implementation of Individual Goal/Habilitation Plans.
- Provides training and assistance in area's of ADL's with participant as designated in the Individual Plan. (including; bathing, grooming, toileting, dressing, laundry, etc.).
- Carries out responsibilities as assigned (i.e., doctor's appointments, church attendance, banking, counseling sessions, grocery/personal shopping, etc.).
- Foster independence and autonomy.
2. Community Integration
- Provide updated information and access to neighborhood/community resources aimed at full "inclusion" to the participant.
- As opportunities arise, expose participant to a variety of functions/activities to increase choice making.
- Communications with Program Manager in regard to implemented integrated activities and any issues that may arise.
3. Crisis Management
- Utilizes pro-active approach to avert crisis situations.
- Utilizes on-call supervisor to request emergency assistance.
4. Professionalism
- Participates in participant meetings as needed.
- Develops an appropriate rapport with participants.
- Keeps written and verbal information shared with team members regarding a participant confidential.
- Demonstrates sensitivity and responsiveness to participants and respects confidentiality needs.
- Wears attire appropriate to the working environment and situation.
5. Documentation
- Documents all paperwork in a clear, objective and professional manner (i.e., Incident Reports, accident reports, daily summaries, etc.) following established guidelines.
- Timesheet to be completed and initialed daily. Original timesheet to remain at participants home at all times.
6. Basic Health and Safety
- Completes certification in CPR and First Aid.
- Is oriented to fire procedures and disaster plan in participant homes.
- Maintains home so as to ensure safety and cleanliness on a daily basis.
7. Meal Planning and Preparation
- Train and/or assists participants in meal preparation to promote skill building and independence. May include feeding.
- Will follow dietary guidelines as directed in Habilitation Plans.
8. Transportation
- Reports any accidents or traffic violations, minor or major, immediately to Program Manager.
- Drive according to New York State Laws.
- Uses participant vehicle in a responsible manner.
- Uses participant vehicle for job related business only.
- Follow individual Van contract guidlelines.
9. Housekeeping
- Completes daily housekeeping tasks with assistance from participants, based on individual need.
10. Pet Care
- Perform pet care as designated by participant (i.e., feeding, walking, etc.).
11. Perform other duties as assigned.
Scope of Responsibilties
With on-going supervision, provide training and assistance to individuals living independently or with family with the intent of encouraging independence and inclusion.
Education and Experience
High school diploma/GED. Experience working with people with disabilities preferred.
Conditions of Employment
Must have a valid NYS driver license in accordance with the Agency Vehicle and Driving Policy. Must be able to lift 75 pounds. Successful completion of required in-service training. Must participate and meet established requirements in a database check through the Division of Criminal Justice Services as defined by the State of New York, Office of Mental Retardation and Developmental Disabilities, Chapter 575 of the Laws of 2004. Must participate in successful database checks through the State Central Registry of Child Abuse and Maltreatment, HHS Office of Inspector General and Excluded Parties List System and OMRDD Criminal Background check.
